What You’ll Need
Opening your Representative Payee Account is easy. Just bring the following items into your nearest Ridgewood branch:
- Your Photo ID
- Your Social Security number
- Proof of your New York State address
- The beneficiary’s Social Security number and date of birth
You’ll also need to provide one of the following:
- A letter of appointment from the Social Security Administration (SSA)
- A check made out to the representative payee
